Release of Liability and Consent Form

Tiffany Campbell / Healing with Tiff, LLC is providing a service through the use of modalities that may include, but not limited to: Law of Attraction Coaching, Reiki Treatment, Emotion Code, Body Code, QRNR (Quantum Release Nutritional Roadmapping), AO Scan Technology, Global Energy Method (GEM), F.I.X. Code, Meditations, Chakra Balancing, Acupressure, Sound Therapy, Physical Energy Balancing, or other Alternative Healing Modalities.

You understand that Tiffany Campbell / Healing with Tiff, LLC employs complimentary healing modalities that in no way substitute for medical interventions, body therapy, or psychotherapy. You acknowledge that open communication is encouraged between Tiffany Campbell / Healing with Tiff, LLC and you to enhance a mutual understanding and acceptance of the energy work provided during any session.

I, the participant, hereby agree to assume all risks associated with any outcome resulting from my participation in Tiffany Campbell / Healing with Tiff, LLC’s programs, group or private sessions. Side effects may include, but not limited to: emotional release, lightheadedness, mild fatigue, physical tingling, and change in body temperature which may occur even weeks after the session.

I hold Tiffany Campbell / Healing with Tiff, LLC harmless from any possible effects that may cause temporary physical or emotional discomfort and agree to take full responsibility for my self-care and personal development. I am in control of my own body and I can always ‘stop’ at any time.

It is my responsibility, as a participant, to inform Tiffany Campbell / Healing with Tiff, LLC if any discomfort should arise. It is my responsibility, as a participant, to notify my practitioner ahead of treatment of any pre-existing medical conditions or injuries and secure clearance to participate.

By signing this Waiver & Release of Liability, I agree that Tiffany Campbell / Healing with Tiff, LLC shall not be responsible for any negative consequences of any kind which is claimed to result from: 1. My participation in any programs, activities, or sessions; or 2. From the use of any information learned or obtained during these programs, classes, or sessions.

By signing this Waiver & Release of Liability, I further agree not to assert any legal claims of any kind in any form against Tiffany Campbell / Healing with Tiff, LLC on any asserted negative consequence resulting from my participation in these programs, classes, or sessions.

By signing this Waiver & Release of Liability, I acknowledge and agree that I alone am responsible for my health and wellness and any outcome of the programs, classes, or sessions, Tiffany Campbell / Healing with Tiff, LLC disclaims all liability for any injuries or negative consequence claimed by me to be the result of my participation in these programs, classes, or sessions.

I have been encouraged by Tiffany Campbell / Healing with Tiff, LLC to seek the advice of legal counsel concerning this Acknowledgement and Waiver of Liability; and I execute and deliver this Acknowledgement and Waiver of Liability freely and voluntarily and without duress or coercion and with full knowledge of the representations contained herein and the rights relinquished, surrendered, released, and discharge hereunder.

No guarantees can be made by Tiffany Campbell / Healing with Tiff, LLC programs, classes, or sessions and it is recommended that you seek medical treatment and/or evaluation for any medical concerns. Programs, classes, or sessions are not a substitute for medical treatment or medical evaluation and are not intended for clients who are having a medical crisis.

My signature also indicates that I am agreeing to the cancellation policy of Tiffany Campbell / Healing with Tiff, LLC that requires a 24-hour cancellation notice by email to [email protected] or I will be responsible for the full payment of the program, class, or session.
